What we ask for when you enroll
You buy the plan on this site. There is no separate application.
For each person, enter the full name, choose male or female, and enter the birthdate, phone number, and legal residence: street, city, state, and ZIP. Choose funeral or cremation. The couples plan asks for both people.
Then pay. $495 for one person, $990 for two. The plan is not active until payment is received and an enrollment number is issued. If you buy it while you are away from home, it starts after you return and travel again.
